Suppport Coordination
Level 1 and 2

Support coordination is not about choosing services for you—it’s about empowering you to make informed decisions, navigate the NDIS, and build resilience and independence.
There are three levels of support coordination available depending on your needs:

Support Connection (Level 1): Helps you understand your plan and connect with supports

Coordination of Supports (Level 2): Assists with designing and managing supports

Specialist Support Coordination (Level 3): For participants with complex needs requiring expert coordination
What is Support Coordination?
Support coordination is a capacity building support funded in your NDIS plan. Its purpose is to help you understand, implement, and make the most of your supports.
A support coordinator works with you to:

Understand your NDIS plan and what supports are funded

Connect you with providers, community, mainstream, and government services

Build your skills and confidence to manage your supports independently

Design and coordinate support approaches tailored to your goals and needs

Establish and maintain service agreements with providers

Monitor your plan budget and the effectiveness of supports

Prepare for unexpected situations, including crisis planning and response

Coach and reflect with you to refine your support strategies over time

Report to the NDIA as required, maintaining accurate documentation
